I have noticed this on a variety of occasions with varying line differences.
Any thoughts on cause and solutions?

It just blends in extremely well. It is the first line of text, above “Note the first image is the true count of the relevant code. The second image contains the lines that Gemini Pro Preview 3 thought the snippet was on.”.
It looks slightly highlighted. It is a snippet from the AI Studio Prompt output, and mentions replacing lines 227-232. Hope this helps.
